Be careful. If you want to make an insurance claim, ask for a quote first. Unfortunately I did not do that and they charged me a much higher cost than is prescribed in the Ontario Dental Association Guidelines. Obviously, the insurance company did not want to cover the whole cost of the procedure, only the part mentioned in the Guidelines. When I called Dr. Martin's assistant, she told me that's their rates and they wouldn't give a discount based on Guidelines, as they don't follow them (i.e. the Guidelines).
