Page Content The South East Community Care Access Centre is mandated to provide community-based health services, Long-Term Care Home Placement Coordination and Information & Referral services to the almost 500, 000 residents of the South East. With offices in Belleville, Bancroft, Kingston, Northbrook, Selby, Brockville and Smiths Falls, a presence in 14 sites of 7 local hospitals and 3 Family Health Teams the South East CCAC provides services to more than 11, 000 clients on any given day. In 2012/2013, South East CCAC provided care to more than 33, 410 people including: 20, 889 patients receiving care at home 8, 618 patients came from community referrals, 12, 071 patients came from hospitals 14, 481 seniors who were supported to stay in their homes independently 3, 451 children receiving health services at school Coordinated care for an average of 3, 283 people each day In homes, schools and communities across the South East, the CCAC provided: 300, 606 nursing visits and 31, 304 hours of shift nursing 1, 282, 537 hours of personal support services Provided 663 more patients with personal support, and 100, 248 more hours of personal support than the previous year 65, 980 visits from rehabilitation professionals (physiotherapists, occupational therapists and speech language pathologists) 4, 158 social work visits and 2, 472 visits from clinical nutritionists/dietitians Long-Term Care Admissions There are 36 Long-Term Care Homes throughout Southeastern Ontario. Assisted more than 1, 242 seniors transition to a long-term care home Waitlist information is posted on our public website on the 15th of every month.
