ESWT - Low Energy Extracorporeal Shock Wave Therapy

ESWT or Extracorporeal Shockwave Therapy

Extracorporeal shock wave therapy (sound waves) has become a ground-breaking therapy in orthopaedics within the last few years. With the number of indications increasing, ESWT presents an effective alternative to surgery and the protracted therapy often associated with it. The shock waves used in the treatment are high energy acoustic sound waves generated outside the body. The waves are precisely focused and transmitted through a water cushion into the patient’s body where they produce a therapeutic effect. This procedure is a conservative and non-invasive therapy method that creates no scars and is conducted on an outpatient basis, normally without anesthesia. The efficacy of ESWT has been demonstrated in the treatment of numerous conditions. The results of new evidence-based studies show an even greater spectrum of ESWT applications.

Some of the Most important indications for ESWT

Plantar fasciitis, or heel spurs, is one of the most common ESWT indications. A heel spur is a deposit of calcium where the plantar fascia attaches to the heel and the aponeurosis. It can develop due to an overstraining of the tendon. The plantar fascia is the broad tendon on the bottom of the foot which supports the arch of the foot and protects sensitive structures such as nerves, blood vessels, muscles and other tendons. In many cases there is chronic inflammation of the tendon which leads to pain without a real spur. Through the use of shock wave treatment it is possible to heal the inflammation thereby enabling the patient to walk again without pain.

Calcified shoulder, or tendinosis calcarea, is a painful inflammation or acampsia of the shoulder joint capsule which is often caused by, or associated with, calcium deposits. Typical symptoms are unendurable pain during the night or if the arm is moved and difficulty lifting the arm sideways. Through the use of shock waves it is possible to heal the chronic inflammation and enable the patient to move the arm without pain. In many cases a disbandment of the calcium deposits can also be observed.

Tennis elbow is a common and painful inflammation of several structures of the elbow, including the muscles, tendons and periosteum. This disease is caused by repeated forceful contractions of the wrist muscles usually located on the outer forearm as well as chronic stress on tissues attaching the forearm muscles to the elbow area. The term “tennis” is therefore just representative of the various types of possible causes. Typically when the patient grips something there is a strong pain, which goes from the elbow to the hand.

Another application for ESWT is pseudarthrosis or delayed healing of a bone fracture. As the result of a fracture, in some cases, the body generates a chondral union instead of a bony union. This is a painful and limiting condition. The general form of treatment is surgery where bone tissue is often transplanted. ESWT represents a non-invasive treatment alternative which stimulates the tissue to generate new bone cells.

Potential side effects of ESWT After treatment with shock waves, pain can increase for a time and swelling, localized numbness or tingling can occur. Hematoma and small purplish spots on the skin are also possible. All of these side effects are temporary and resolve in time.

Something you should know about Low Back Pain

Low back pain affects 85% of the working population and 80% of Canadians will suffer from low back pain in their lifetimes. In a recent Environics poll, nearly two-thirds of adults in Canada said they experienced moderate to severe back pain in the past year, causing them to lose anywhere from a day to more than a month off work.

Health Canada estimated that musculoskeletal disorders, including back pain, cost society $16.4 billion in combined direct (treatment and rehabilitation) and indirect (lost productivity) costs.

Back Pain is a leading cause of prescription (and non prescription) drug use, medical visits, hospitalizations and restricted physical activity. In Ontario alone, back pain accounts for one-third of lost work claims, according to the Institute of Work and Health.

The main causes of back pain are: improper lifting, carrying heavy articles, sports injuries, poor posture, health conditions and work related injuries.

Many ordinary activities like working at a computer and driving can lead to a posture that puts additional pressure on your lower back. By the time you feel pain, the problem has most likely been in the making for some time.

Actually, in 1999 Canadians spent $21.5 million on back pain products. Yet while many of these avenues offer relief, they may not get to the root of the problem.

Back pain affects Canadians of all ages and slightly more women than men. Their effect has the greatest impact on adults under 45, and is only second to arthritis in those 45 to 65 years of age. For them, it’s the number one cause of limited activity, keeping them from work, out of the game and away from the garden.

The findings also show that almost half the people asked said that coping with back pain made it more difficult to concentrate on everyday tasks–which means not just dealing with the pain, but also having to manage the disruption it causes in their life and the lives of those around them.

While some people treat only their pain, others ignore it altogether, dismissing its severity or thinking it will just go away. But because so many people’s everyday activities contribute to the problem, backs are routinely and repeatedly exposed to risks. And the reality is that ignoring the problem doesn’t make it go away. In fact, it can make it worse.

For Canadians suffering from back pain, getting back on their feet means taking an active role in treating not just the symptoms, but also the problem. And to help them, chiropractors are able to offer many different treatment options.

Common Questions about Low Back Pain
How do you get Back Pain?
One of the most common causes of low back pain (LBP) is an overuse syndrome that creates muscular pain. However, other significant causes of low back pain include direct trauma, lifting and twisting, age-related changes creating degeneration, and postural stresses.

How is it treated?
Chiropractors are practitioners of choice when it comes to back pain, and have a high rate of patient satisfaction. About 4 million Canadians consult a chiropractor each year.

Joint manipulation and home-based exercises are the best first line treatment options for most causes of LBP; a supervised chiropractic program is beneficial.

Three recent reports have concluded that increased utilization of chiropractic care can contribute to improved productivity and reduce benefit costs.

As well, patients treated by a chiropractor for back pain returned to work sooner and had reduced rates of chronic problems and hospitalization.

Reports demonstrate that chiropractic is more effective – and cost-effective – care for occupational and non-occupational back injuries and related conditions than other common forms of treatment. And chiropractic can play an even greater role in improving organizational health and productivity, and reducing absenteeism and disability.

Is lower back pain really a serious problem?
First of all, if it makes life more difficult for you, that’s serious in itself. Even if it goes away in a few days you haven’t really gotten rid of the problem. It will most likely return and if left untreated, could be more debilitating the next time, which is why we recommend seeing a chiropractor at the first sign of back pain.

Lower back pain can also be accompanied by a variety of symptoms, some of which indicate very serious problems. Do you currently have or have you ever had:

Myth
“Chiropractic treatment involves cracking my spine”

Fact
One part of Chiropractic treatment involves applying quick, precise pressure to the joint, which creates a cracking or a popping noise. This is called an adjustment. The popping noise doesn’t come from your bones, but from the release of a gas bubble when two joints are separated. This is just like when you pop your knuckles, or you scrunch up your toes first thing in the morning.

Myth
“No one should use so much pressure on my spine…that can’t be right.”

Fact
The adjustment does not involve a great deal of force. Skill, positioning, speed, and timing are more important than force.

Myth
“I heard that cracking your joints can cause arthritis?”

Fact
There is no reason to think there is a connection. In fact, an adjustment increases mobility, reduces pressure on the joints, muscles, and nerves, and allows the spine to return to a natural position and movement.

A NATURAL APPROACH TO CROHN’S DISEASE

Your digestive system is essentially a long tube that begins at your mouth and ends at your other end. It has one purpose, which is to break down food and extract energy and nutrients from it. Your digestive organs – esophagus, stomach and intestines – are simply modifications of this tube.
This system if very efficient as long as it is functioning normally. Many people, however, suffer from a malfunction of their digestive tract. In fact, the three most common conditions affecting intestinal function I see in practice are, irritable bowel syndrome (IBS), ulcerative colitis and Crohn’s disease. Although not the same, they all have a great deal in common and escalate in severity.
Crohn’s disease, the most serious, is a very chronic long- term ulceration of the digestive tract. The condition itself extends through the wall of the intestine and involves all layers of your “digestive tube”.
Ulcerative colitis on the other hand involves mainly the internal intestinal layers called the mucosa and the submucosa layers. Patients affected by this condition suffer from chronic diarrhea, rectal bleeding, abdominal cramping, pain, fever, fatigue, loss of appetite, loss of weight, malabsorption, malnutrition and headaches. Crohn’s disease can be seen in the early teens to early 30s. If left untreated, bowel function gradually deteriorates. This can be life threatening. It has been noted that Crohn’s disease increases the risk of colon cancer by as much as 20 times. This is also true for ulcerative colitis and to some extent irritable bowel syndrome. Definitely conditions that should be avoided at all cost.

Chiropractic – Holistic Approach
Chiropractors tend to approach people suffering with irritable bowel syndrome, ulcerative colitis, or Crohn’s disease from a different perspective than the family physician. We use a holistic approach.
One needs to remember that we are dealing with individual human beings and not only their intestines. Many patients, for instance, are surprised to learn that the body is a self-healing and self- regulating organism. This means that your body is designed to heal itself and to regulate its own functions. You’re “on auto-pilot”. In other words, the cells in your digestive tract deteriorate continuously and are replaced by brand new cells doing the same function as the originals. This “auto-pilot” is your nervous system, your own Internet! It is similar to the NASA control center and coordinates and controls every single function of your body. As long as it is able to control your body with no interference, you should have the best health possible. If, however, there is some sort if interference to the normal function of your nervous system, your body will simply work the way it was designed.
In Crohn’s disease, the intestinal cells deteriorate but are not necessarily replaced by new ones. Over the course of time, the digestive system essentially deteriorates. A number of studies have actually shown that Crohn’s disease and ulcerative colitis are essentially neurologically based. This means that the nervous system is no longer able to control the normal function of the intestinal area.
Chiropractors are mainly concerned with nervous system function. We check the spine to make sure there is no interference to the normal workings of nerves, which control your digestive system. Our profession has had a great deal of success in helping people with these three afflictions of the intestinal tract.

Supplement Support
As well as seeing your chiropractor, which I feel is essential; there are a number of things that you can do to help your body and to help your intestinal area function better. A hot water bottle placed on your stomach when you are experiencing severe cramping will offer considerable relief. Here are some other recommendations:
Charcoal tablets will help absorb excess gas for those who experience bloating. These are not to be used for a prolonged time because they also absorb most of the nutrients in the digestive system.
Slippery elm powder can be of considerable benefit. It tends to relax the muscles of the intestinal wall and ease cramping.
Multi-vitamin and mineral complex that contains copper, manganese, selenium and potassium. Crohn’s disease is characterized by intestinal malabsorption and these elements are essential.
Vitamin A, approximately 50,000 IU of daily, unless of course you are pregnant. You may take this in a capsule form, but I suggest that you put raw peeled carrots through a juicer and have pure carrot juice every single day. The reason I suggest carrots be peeled is that the skin contains most of the chemicals and additives which can irritate your bowel.
Vitamin E. Vitamin A and E are essential for repair of the epithelial tissue of the intestinal tract.
Calcium and magnesium, at least 2,000 mg daily because of the malabsorption one may be experiencing
Vitamin C in very high doses combined with bioflavanoids is essential, as this prevents inflammation and increases your immune response. I suggest at least 5,000-mg. Per day, taken in a buffered preparation, so that it does not upset your stomach.
Garlic capsules are of great benefit in that they prevent free radicals from attacking the part of the intestine, which is inflamed. Garlic has also been known to aid in the healing process.
Vitamin B12 and folic acid are crucial in dealing with Crohn’s disease. These are poorly assimilated from the gut, so you may want to see your physician regarding having regular injections. These may also be taken in a sublingual form under your tongue.
N-acetylglucosamine (NAG) is also important in that it protects the intestinal lining from the enzymes that could damage it. Otherwise, you would literally digest yourself!


Glutamine, which is an amino acid, should also be taken with juice or water, not milk, and the absorption of it is enhanced when vitamin C is taken at the same time.
The solution is to maintain the intestinal lining that absorbs nutrients from the food that you eat. Taking an amino acid complex is also a good idea, as it tends to soothe the intestines.

Dietary Solutions
Drink lots of fluids. Steamed or distilled water is excellent; clear, fresh spring water is also wonderful. Herbal teas and fresh juices are terrific. You may not like this next one; however, cabbage juice is wonderful! And of course I have already recommended carrot juice. Chewing papaya seeds will be helpful, as they contain enzymes, which may aid in digestion.
Eliminating all dairy foods from your diet will be of tremendous help. Dairy foods are difficult to digest and they are mucous producing. Avoid alcohol, caffeine and carbonated beverages such as Pepsi or coke. You probably don’t realize this but if you spill a can of coke on freshly waxed floors, it will eat the wax right off the floor. Imagine what it does to your insides.
Chocolate, eggs, foods with artificial additives or preservatives, or fried and greasy foods, as well as spicy foods and tobacco, are very irritating to the digestive system. As well, try to avoid refined carbohydrates, such as boxed cereals, cookies and cake.
I recommend a good diet of non-acidic, fresh vegetables, raw or slightly cooked such as broccoli, brussel sprouts, carrots, celery, turnips, spinach and cabbage. You can steam, boil, bake or broil all of these- it really doesn’t make much of a difference. However if you boil them for a long period of time, you destroy the nutritional value of most foods.
You also need to remember that anyone suffering from Crohn’s disease has a general nutritional deficiency, and may require approximately 30 percent more protein than normal. If chronic diarrhea is present, you also need extra electrolytes to balance your body chemistry. Corticosteroids or anti-inflammatory drugs tend to suppress protein synthesis and inhibit normal calcium absorption by increasing excretion of vitamin C in the urine. For this reason, I suggest you take multi-vitamins to balance your chemistry.
It is important that you stress as much as possible. Our mental states tend to control our physiology and are deeply intertwined with our body functions. If you are suffering from an acute attack, it is important that you have rest and peace of mind. This will be of tremendous benefit in helping your body deal with this condition.
